Provides funding for tribal and urban Indian health programs to train and employ Community Health Representatives (CHRs). CHRs provide basic health education, case management, and referral services to American Indian and Alaska Native communities.

- Must be a federally recognized tribe, tribal organization, or urban Indian organization
- Must have a healthcare delivery system
